FOR DOCTORS OF IMA - A.P. STATE UNIT

Coverage z Expenses incurred towards hospitalization for treatment of the following diseased are covered.
1) All Cardiac Disease 2) All forms of Cancer
3) Kidney Failure inculding renal transplant 4) Paralysis and Stroke
5) Any Accidental Injury 6) Encephalitis
7) Neuro Surgery 8) Total replacement of joins &
9) Liver disorder associated with complications like cirrhosis of liver

Sum Insured : Rs. 1,50,000/- per person per year.

Period : Long Term Policy for 5 years
Age Limit : 5 years to 80 years

Who are Eligible: Members of IMA, their spouse and dependant children

Additional : (Within the limits of sum insured i.e., 1,50,000/-)

Benefits : Pre-hospitalisation expenses incurred during period upto 30 days prior to Hospitalisation.
Post-hospitalisation expenses incurred during period upto 60 days from the day of discharge from the Hospital.
Exclusions : + Pre-existing diseases.
. Hospitalisation during the first 30 days of commencemnt of po’icy except those arising out of accident.
+ Domiciliary Hospitalisation.
• Cost of vitamins, tonics and expenses relating to naturopathy convalescence, general debility, pregnancy etc.
+ Diagnostic charges etc., not consistent with or incidental to the treatment.
Claim Procedure:
+ A preliminary notice Of claim with particulars relating to name of the insured person in respect of whom claim is made may be sent directly to the insurance company with a copy to IMA with address of the hospital I nursing home within 7 days of hospitalisation.
+ Claim form duly filled-in, discharge summary along with medical prescription with diagnostic reports and cash bills such other documents related to hospitalisation within 30 days from the completion of treatment and also advice for hospitalisation.
